Highly Specific Paper-based Detection of Neonatal Sepsis

Summary
Researchers at Purdue University have developed bench-top molecular biology detection into a portable and easy-to-use paper test strip. Assay time is decreased (less than 1.5 hours) by adjusting LAMP reagent concentration, and its high selectivity decreases the amount of false positives. In addition, this detection strip is within the amplification reaction itself and does not require downstream steps to perform detection. The paper effectively prepares, amplifies, and detects bacterial nucleic acids, such as E. coli, from infected whole-blood samples for rapid sample-to-answer disease detection.

*Background
In low-middle income countries, 42 percent of neonatal deaths are caused by bacterial pathogens entering the blood stream and causing sepsis infection. Diagnosis of early onset neonatal sepsis currently requires 24-48 hours of blood culture incubation and infrastructure that is frequently inaccessible for delivery bedside or to hospitals of developing countries. Isothermal nucleic acid amplification using a technique called LAMP (loop-mediated isothermal amplification) is a highly sensitive method for increasing the detection rate of specific gene targets in paper-based rapid diagnostic tests. However, this detection method currently only measures the total amount of nucleic acid generated, not a specific target, which causes the tests to produce false positive and false negative results. There is need of a method for specific target detection to integrate into LAMP diagnostic tests.
