Each year, a significant number of code blue and emergent transferevents occur in pediatric hospitals. Current prediction methods do not have ahigh level of success with alerting medical staff to an imminent code blueevent. Researchers at Nationwide Children's Hospital have developed analgorithm for calculating a pediatric early warning index score designed tocorrelate with the risk of cardiopulmonary failure and/or emergency transfer tothe ICU based on only objective vital sign measurements. The Vitals Risk Index(VRI) alerts medical staff when early warning index scores exceed a specifiedthreshold. VRI performs as well as or better than current measures such as thePEWS score system and Rothman Index. Additionally, the VRI requires lessskilled nursing effort to predict and alert possible adverse events. The VRIalgorithm has been validated in a retrospective study and is currently beingimplemented in the Nationwide Children’s Hospital Emergency Medical Recordsystem to facilitate a prospective evaluation. If implemented in hospitals,this technology has the potential to help prevent code blue events and increaseemergency medical staff preparedness.
