Innovative Methods for Multi-Modality Image Adaptive Radiotherapy & Replanning

技术优势
- Reduced risk of excess radiation to surrounding healthy tissues- Increase in the amount of radiation delivered to the target tissue- Determination of dose gradients from dose distribution in original plan for all critical structures near the target: replanning optimization runs to maintain the originally planned dose gradients (unlike regular optimization methods that rely on dose-volume criteria which vary drastically from day to day due to the variation in the volume of organs like the rectum and bladder)- Enables automated delineation based on MRI or CT- Allows use of only MRI for radiation treatment planning & drastically reduces replanning time- On-line correction (unlike off-line) strategies correct for both systematic and random variations- On-line correction (unlike off-line corrections) apply to a therapy course with a small number of treatment fractions, such as hypo-fractionation or stereotactic body RT (SBRT)- Generation of the daily OAR is not required with Gradient maintenance (GM) & makes daily re-planning more reproducible & eliminates necessity for trial & error effort

*Abstract
Adaptive Radiotherapy (ART) is a state-of-the-art approach that can deliver highly individualized ther-apy for cancer patients. This invention embodies three novel methods for significant improvement of the ART process by improving the accuracy and speed:- An acquisition/processing method to generate uniform MR image intensities, improving tissue classification, DIR, and synthetic CT generation.- Generation of synthetic CT from MRI by combining MRI relaxometry parameter maps with atlas based tissue classification that are not confounded by B0 or B1 inhomogeneities/distortions - A fast replanning algorithm for fixed- or rotational-beam, or combined, delivery based on two steps: creating/adjusting beam/segment apertures based on target of the day, and optimizing beam/segment weights using automatically generated ring structures on the image of the day.
