Since HIF-1 is a transcription factor that controls the expression of multiple genes involved in angiogenesis, it will provide a superior clinical outcome compared to treatment with a single angiogenic factor (such as vascular endothelial growth factor, for example). Further, since the HIF mutein resists degradation in nonhypoxic tissue, one may prophylactically treat tissue that may become hypoxic at a later time. For example, a narrowed coronary vessel may become completely occluded either by spasm or a clot, resulting in a myocardial infarction (heart attack). One may use the stable HIF mutein to induce angiogenesis in these patients, even if there is no hypoxia at the time, in order to prevent heart attacks.

Hypoxia occurs when the demand for oxygen in a tissue exceeds the supply. Hypoxia inducible factor-1 (HIF-1) is a mammalian transcription factor that is expressed in response to hypoxia; further, hypoxia acts to increase the steady-state levels of HIF-1since the protein is prone to degradation in the presence of oxygen. This technology provides a HIF mutein that is more resistant to degradation (as compared to the wild-type protein) under nonhypoxic conditions. This translates into an increased half-life for the mutein in nonhypoxic conditions, and opens several new possibilities for therapeutic and prophylactic treatment of hypoxia and ischemia related tissue damage, as well as for modulating angiogenesis or vascularization.
