The Probability of Repeated Admission (Pra TM) survey identifies senior citizens who are at high risk for repeated hospital admissions and extensive use of health care services. Early prediction of repeated hospitalization allows medical providers to adjust patient care, resulting in better patient health and lowered expenses for hospitals and senior health insurance programs. The survey consists of a health questionnaire, a software-based scoring formula to interpret the survey responses, survey instructions for mail and telephone administered surveys, and general usage instructions.
